Volunteer applicants over the age of 16 are asked to provide an up-to-date CV and references. To comply with the Disclosure and Barring Service (DBS) guidelines, volunteers aged 16 years and over are also asked to provide an Enhanced DBS certificate (with barring). Via services provided by partner organisations, we can help or provide advice on applying for an enhanced DBS certificate.

The information which you give when completing this form and subsequent registration documents, will be used in accordance with the Data Protection Act 1998 for the following purposes: to enable The Roger Diamond Foundation (RDF) to create an electronic and paper record of your application; to enable the application to be processed. The information you give will be kept securely, and will be kept no longer than necessary.
